Prisoner at Saskatchewan Penitentiary dies

For the second time in weeks, an inmate has died while in custody at Saskatchewan Penitentiary.

On Wednesday, emergency services were called to provide assistance to Daniel James Tokarchuk, a prisoner at the federal institution just west of Prince Albert. He was taken to hospital and pronounced dead at 4:24 a.m.

Tokarchuk, 44 years old, had been serving an indeterminate sentence since December 16, 2004 for second degree murder.

His next of kin have been notified of his death.

Correctional Service Canada said in a press release that they will review the circumstances of the incident. The police and coroner have been notified.

Another inmate, Curtis Robert Cozart, died while incarcerated at the institution on May 24.
